[A] Budget: Looking for Proposal
[B] Scope of Service:
Vendor needs to provide medical laboratory services to the government authority located in Jefferson, AR.
- The Contractor shall provide supplies and staff for scheduled, on-site monthly or semi-monthly phlebotomy, for approximately three hours per month.
- The Contractor staff shall transport samples from NCTR to the contractor's own facility for lab analysis and results.
- Contractor staff shall contact occupational health nurse and/or physician with any panic level results as noted by the contractor's lab protocols.
- Contractor staff shall provide, either directly or by subcontract, any non-routine laboratory services deemed appropriate by the physician.
- Specific laboratory services performed under this contract will include but are not necessarily limited to the following: blood chemistry analysis, chemistry panel, CBC, Lipid panel, and TSH. In addition, the laboratory services will provide PSA, measles titer, Oxford Spot test and chest x-rays.
- Related services may be required of the Contractor if ordered by the physician on an as-needed-basis. X-rays shall be performed at the contractor's facility.
