(1) Vendor needs to provide homeless outreach services for targeted community development initiative to the government authority located in Little Rock, AR.
- Creating a comprehensive plan, timeline, and plan for implementation of Homeless Outreach Services to serve unsheltered persons in these areas:
1. The vendor shall provide services within the designated area, which will allow access to Homeless services, to include housing assistance, health care services, social service and wrap-around service options in close proximity to assist at minimum 250 individuals within a 12-month period.
2. The vendor shall in put into HMIS all persons who are provided services.
3. The vendor shall collaborate with Central Arkansas Team Care for Homelessness (CATCH) by attending meetings and sharing homeless service data through HMIS.
4. The vendor shall provide standards, boundaries, and guidance for positive outcomes with service assessment, protocol, and timelines to service the intended target audience. Some of this research and best practices may be obtained through site visits to other similar locations.
5. The vendor must assist with the Point in Time Count (PIT) for unsheltered individuals and families. This information shall be reported to the appropriate person under CATCH.
6. The vendor shall use the Built for Zero methodology and participate with CATCH on developing the Coordinated Entry process.
