[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide therapy services to the government authority located in Little Rock, AR.
- Physician’s orders shall be required before vendor can provide any service other than screenings to patients.
- Provide therapy services to adult and adolescent patients of agency as needed.
- Provision of services must comply with centers for medicaid and Medicare services (CMS) and the joint commission (TJC) standards concerning the provision of specialized rehabilitation services.
- Therapists must perform functions as specified by agency, in accordance with all federal, state, institutional procedures, policies, guidelines, administrative orders, directives and applicable regulations. Including HIPAA (health insurance portability and accountability act) compliance as applicable to the agency location.
(2) All question must be submitted no later than November 23, 2021.
(3) Contract term will be one year.

[D] Work Performance:
Performance of the work will be offsite.
