[B] Scope of Service:
Vendor needs to provide integral part of the overall contact tracing strategy within Anchorage to investigate, trace, and monitor the COVID-19 positive cases and their identified close contacts to ensure self-isolation, quarantine and testing of symptomatic contacts to the government authority located in Anchorage, AK.
- Documentation of all encounters and information will be collected on hard copy forms or in a database as directed by AHD point of contact. AHD will provide the format or information on the use of forms. The crew will also be tasked with providing COVID-19 guidance information, employment, or all-clear letters to cases as requested or needed and will be required sign a MOA- AHD confidentiality agreement Attachment.
- Conduct contact monitoring within the Municipality of Anchorage to ensure self-isolation, quarantine and testing of symptomatic contacts. Contractor shall conduct monitoring through calling, texting, and emailing case or contacts on prescribed frequency (as much as 2x a day to in some cases every 3 days) to check and inquire about COVID-19 symptoms and fever.
- Nurse Coordinator: Coordinate, supervise and manage all aspects of the contact monitoring crew activities to include communication, documentation, medical consultation, problem resolution and referrals for those being monitored in consultation and coordination with AHD Medical Officer, Community Health Program Manager and Division Manager as required and needed.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
