(1) Vendor needs to provide life insurance services to the government authority located in Conway, AR.
- Upon attainment of age 65 the amount of life and AD&D insurance shall be reduced by 35%. Upon the attainment of age 70 insurance shall be reduced an additional 15 percent to 50 percent of the original amount. Upon retirement all life and AD&D benefits shall terminate.

Performance of the work will be Onsite. Vendor needs to carry work in their office premises.
